{"id":640,"date":"2015-08-03T15:09:25","date_gmt":"2015-08-03T20:09:25","guid":{"rendered":"http:\/\/www.fhps.net\/?page_id=640"},"modified":"2025-02-19T08:19:57","modified_gmt":"2025-02-19T13:19:57","slug":"facilities","status":"publish","type":"page","link":"https:\/\/www.fhps.net\/departments\/operations\/facilities\/","title":{"rendered":"Facility Use Information"},"content":{"rendered":"<h2>***AT THIS TIME, WE ONLY RENT TO NON-PROFIT\/TAX EXEMPT ORGANIZATIONS***<\/h2>\n<h2><\/h2>\n<h2 class=\"district-thin-rounded-leaf-heading\">Facility Use Forms<\/h2>\n<ul>\n<li><a href=\"https:\/\/www.fhps.net\/documents\/facility-use-application.pdf\" target=\"_blank\" rel=\"noopener\">Facility Use Application Form <i class=\"fa fa-file-pdf-o\"><\/i><\/a><\/li>\n<li><a href=\"https:\/\/www.fhps.net\/documents\/facility-use-exempt-organization-registration-form.pdf\" target=\"_blank\" rel=\"noopener\">Exempt Organization Registration Form <i class=\"fa fa-file-pdf-o\"><\/i><\/a><\/li>\n<\/ul>\n<p style=\"margin-top: 0;\"><em>*Subject to change pending state epidemic orders.<\/em><\/p>\n<h2 class=\"district-thin-rounded-leaf-heading\">Liability Insurance\u00a0Requirements<\/h2>\n<p>Purchaser is required to provide proof of liability insurance coverage by supplying Forest Hills Public Schools with a certificate of insurance.\u00a0 The certificate of insurance is due two (2) weeks prior to the Purchaser\u2019s arrival to campus.\u00a0 The certificate of insurance must address the following detail:<\/p>\n<ul>\n<li>Name Forest Hills Public Schools as the Certificate Holder<\/li>\n<li>Name Forest Hills Public Schools as an additional insured, with the exception of Worker\u2019s Compensation<\/li>\n<li>List liability coverage at no less than $1,000,000 per occurrence<\/li>\n<li>List the policy period as inclusive of the dates covering Purchaser\u2019s stay at Forest Hills Public Schools<\/li>\n<li>List the dates of the activity in the description portion of the insurance certificate<\/li>\n<\/ul>\n<h2 class=\"district-thin-rounded-leaf-heading\">Gymnasium &amp; Field Use<\/h2>\n<div class=\"su-note\"  style=\"border-color:#dbdbdb;border-radius:3px;-moz-border-radius:3px;-webkit-border-radius:3px;\"><div class=\"su-note-inner su-u-clearfix su-u-trim\" style=\"background-color:#f5f5f5;border-color:#ffffff;color:#333333;border-radius:3px;-moz-border-radius:3px;-webkit-border-radius:3px;\">\n<h3><strong>Fall Season<\/strong><\/h3>\n<p><strong>Start:<\/strong> First full day of school.<br \/>\n<strong>End:<\/strong> Last full day of classes before winter break.<\/p>\n<h3><strong>Winter Season<\/strong><\/h3>\n<p><strong>Start:<\/strong> First day of classes after winter break.<br \/>\n<strong>End:<\/strong> Last full day of classes before spring break.<\/p>\n<h3><strong>Spring Season<\/strong><\/h3>\n<p><strong>Start:<\/strong> First day of classes after spring break.<br \/>\n<strong>End:<\/strong> Last full day of class before summer.<\/p>\n<\/div><\/div>\n<p>* Application period is open 6 weeks prior to start of season.<\/p>\n<p>* Facilities will be assigned within 2 weeks of start of season.<\/p>\n<p>NOTE: No access to indoor facilities when school is not in session.
